NDIA Supply Chain Questionnaire

The purpose of the attached survey is to assess the status and attractiveness of the Aerospace and Defense industry to qualified small to medium-sized firms.  As a manager in such a firm, you are being asked to spend about 10 to 15 minutes of your time in completing this survey.  Your responses will help us identify those issues affecting your participation.  The Department of Supply Chain Management, the Eli Broad Graduate School of Management, Michigan State University, in conjunction with the Manufacturing Division of the National Defense Industrial Association (NDIA), is studying the factors affecting the participation of small to medium-sized suppliers to the aerospace and defense industry.  The pool of qualified small to medium-sized suppliers in this industry is shrinking.  Consequently, the NDIA is interested in identifying those issues and concerns affecting the participation of such companies as such yours.

Please note that a completed survey is more useful than a partially completed one. There are no right or wrong answers – only what you think! Your participation is entirely voluntary.  Any data that you provide will be kept in strictest confidence.  No individual responses will be reported; only summarized results will be used and reported. Should you have any questions regarding the items in this survey, please call Steven A. Melnyk at (517) 432-6410 or by email at melnyk@msu.edu.
